1 / WHERE DO YOU OFFER TELEHEALTH SERVICES?
Our practice is 100% remote. Due to state-specific dietetic licensing laws and telehealth regulations, clients must be physically located in Minnesota, California, Arizona, or Colorado at the exact time of their appointment.
2 / DO YOU ACCEPT INSURANCE?
We are an in-network provider for UnitedHealthcare (UHC) of Minnesota. If you are covered under a UHC plan in Minnesota, we will bill your insurance provider directly. For all other insurance plans, our practice operates as an out-of-network provider, and clients are considered self-pay.
3 / HOW DOES OUT-OF-NETWORK BILLING WORK?
For out-of-network clients, payment is required at the time of service. However, through our secure clinical portal (Kalix), we can automatically generate and submit out-of-network claims on your behalf, or provide you with a detailed receipt (a "Superbill"). You may submit this to your insurance provider for potential reimbursement. We highly recommend contacting your insurance provider directly to verify your out-of-network nutrition counseling benefits prior to booking.
4 / WHAT IS YOUR CANCELLATION POLICY?
We require a minimum of 24 hours' notice to cancel or reschedule an appointment. Appointments canceled with less than 24 hours' notice, or unattended appointments, will be charged the full session fee. This policy allows us to offer the time slot to another client requiring care.
5 / WHAT FORMS MUST BE COMPLETED BEFORE MY INITIAL ASSESSMENT?
Upon booking, you will receive an invitation to our secure, HIPAA-compliant client portal. You will be asked to complete a comprehensive health and nutrition intake questionnaire, provide consent for telehealth treatment, and review our standard practice policies. All paperwork must be completed prior to our first meeting to ensure we maximize our clinical time together.
6 / WHAT IS A GOOD FAITH ESTIMATE?
Under the federal No Surprises Act, health care providers are required to give clients who do not have insurance, or who choose not to use their insurance, an estimate of the expected charges for medical services. You will receive a Good Faith Estimate (GFE) prior to your scheduled appointment. For full details, please review the Good Faith Estimate notice linked in our website footer.
7 / ARE MY RECORDS AND PAYMENT INFORMATION SECURE?
Yes. We utilize Kalix, an encrypted, fully HIPAA-compliant electronic medical record (EMR) system. All messaging, clinical notes, intake forms, and billing transactions are securely housed within this portal to protect your privacy.
